Our clinic will be closed from Monday, December 22, 2025, through Friday, January 2, 2026. We will return on Monday, January 5, 2026. Clinic staff will be available on a limited basis to respond to voicemail messages and appointment requests. We hope you have a peaceful and happy holiday season.
We have a patient portal for your convenience. When you enter the portal for the first time, you will be asked to create a log-in and password. After that, you will be able to complete forms for registration as well as securely view and download diagnostic and/or treatment reports from your provider.
