Gifts to the University of Tennessee Health Science Center are made through and payable to the University of Tennessee Foundation, Inc. (UTFI), an independent not-for-profit 501c3 that manages all alumni, development, and philanthropic endeavors of the University of Tennessee and its Health Science Center campus.
Be part of an innovative statewide initiative to improve primary care for all Tennesseans. Your practice can get involved and benefit in several ways:
- Participate in TN Heart Health Network webinars and training opportunities.
- Employ our Heart Health Toolkits for practices, using a patient-centered outcomes research (PCOR) evidence-based approach.
- Learn about new reimbursement opportunities for health coaching and other population health services.
- Get your patients involved in one of our regional Patient Advisory Councils to provide expert guidance regarding the services patients want most.
- Participate in the Tennessee Population Health Data Network (TN-POPnet) and receive quarterly Heart Health Practice Improvement Reports to help you track and improve heart health care in your practice.
The TN Heart Health Network is recruiting patient experts – those who struggle with their weight, high blood pressure, diabetes, or heart disease and those who take care of someone with these conditions – to serve on a Patient Advisory Council (PAC).
These groups meet regularly to share personal experiences and provide feedback to doctors so they can better serve patients. Benefits to joining:
- Contributing to improving heart health care for yourself, your loved ones, and your community
- Connecting with others who are also dedicated to improving care
- $25 per meeting in reimbursement for participation
