About Us

What We Do

Our vision is to improve health outcomes for residents in vulnerable neighborhoods. We do so by providing health coach support and lifestyle/stress management, and by assisting individuals in the community in getting access to essential primary and behavioral health care. The Neighborhood Health Hubs Program works to provide a convenient entry point to primary care for people in high need areas of Memphis and Shelby County. Walk-ins are welcome at all locations.
