Steps to Submit IBC Protocols
Step 1. Complete the IBC Protocol Form.
Download the IBC Protocol Form. Follow the instructions on the form or use the more detailed Instructions to complete IBC Protocol Form. Type directly on this form using any word processing program that is compatible with Microsoft Word.
Step 2. Submit unsigned version for pre-review by the 2nd Friday of the month prior to the scheduled meeting at which you wish to have your registration reviewed.
All registrations are pre-reviewed prior to full committee review.
Step 3. Sign and Date Registration (required signatures include the Principal Investigator and the Department Head).
Note: Only signed registrations will be reviewed and considered for approval by the IBC at its regularly scheduled meetings.
Step 4. Submit the Registration to the IBC office.
You may use either of the following two methods for submission of your completed and signed Registration:
- Forward the original signed registration via campus mail to: Institutional Biosafety Committee, 910 Madison Avenue, Ste. 650.
- Scan and email the original signed registration as a pdf document. Keep in mind that the pdf file must include the principal investigator and the department chair's signatures with dates, and should be of sufficient quality for IBC members to read. Email the pdf to the UTHSC IBC office at firstname.lastname@example.org and send the original signed copy to the IBC office by campus mail.
The IBC meets on the first Friday of each month.
If you wish to have your registration reviewed at the next scheduled meeting, the previewed registration with all administrative signatures should be received in the IBC office no later than 12:00 noon on the 4th Friday of the prior month.
